Take Control of Your Diabetes!
Diabetes Self-Management Education and Support (DSME) Classes
The Gateway District Health Department would like to invite you to join us for diabetes education group classesheld October through December 2019. The 4 classes will be held once weekly for approximately 2 hours and cover different topics every class to help you take control of your diabetes. The DSME curriculum is brought to you by the Kentucky Diabetes Prevention and Control Program (KDPCP) and will be presented by Jason McKenzie, APRN, FNP-C.
Classes will cover topics about:
DM basics and overview
Goal Setting and Behavior Modification
Food and meal planning
Preventing short-term and long-term complications
Bath County Health Department on Thursday October 3rd, 10th, and 24th from 10am-12pm. Class on Thursday, October 17th will be from 1pm-3pm.
Morgan County Health Department on Tuesday October 1st, 8th, 15th, and 22nd from 10am-12pm.
Menifee County Health Department on Wednesday November 6th, 13th, 20th, and 27th from 10am-12pm.
Elliott County Health Department on Friday, November 1st, 8th, 22nd, and Wednesday, December 4th from 10am-12pm.
Rowan County Health Department on Monday, December 2nd, 16th, 23rd, and 30th from 10am-12pm.
Please register with the front desk and let us know you are coming.
Each class covers different material and “flows” from day 1, so it is vital to start the classes on day 1. Therefore, it is important to make every attempt to come for each class! Example, if you do not make it to the initial class and show up for the 2nd or 3rd, you will miss important content that leads into the next classes.
P.S. Don’t forget, individual diabetes counseling available. Just ask front desk staff.
